Clinical Pharmacist (Discharge/Med Rec)

Summary The primary responsibility is to provide pharmaceutical services to the veteran population served by the Pharmacy Service at the HWWVAMC.
Responsibilities Major Duties and
Responsibilities:
Serves as subject matter expert and medical center point of contact for medication reconciliation and pharmacy discharge process.
Coordinates pharmacy discharge program, including collection, analysis and reporting of discharge quality assurance data; development of policy and procedures; and training of other pharmacy personnel.
Works to reduce waste by delaying fills for medications that patient already has at home.
Ensures that medications are left in secure location if patient is not immediately leaving the hospital.
Collaborates with clinical pharmacy practitioners to ensure that patients are scheduled for appropriate follow-up care in pharmacy-managed clinics (PACT, CHF, Anticoagulation).
Performs follow-up phone calls to recently discharged patients to discuss and address any medication-related issues.
Reviews discharge prescription orders not picked up; facilitates pick-up or mail as appropriate if patient went home without medications.
Ensures all window and discharge medications are filled accurately and in a timely fashion.
Provides findings and responses to drug information requests to prescribers in a timely manner and provides appropriate alternatives to current treatment plans as needed.
Investigates and resolves medication discrepancies with the prescribing provider or assigned clinical pharmacy specialist.
Notifying supervisor or other designated personnel of actual or anticipated shortages of medications and supplies.
Receives, interprets and evaluates medication orders for both accuracy and appropriateness using acceptable pharmacy practice standards.

Be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy AND/OR Have a Doctor of Pharmacy (Pharm.

) degree OR Received an advanced degree from an accredited program or are on track to complete advanced degree from an accredited program by the time of hire.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.
D.
program.
A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations.
Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
1.
Knowledge of professional pharmacy practice.
2.
Ability to communicate orally and in writing to both patients and health care staff.
3.
Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4.
Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred
Experience:
1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.
D.
program.

Physical Requirements:
